About this policy
This policy explains how Gilead Medical Associates ("Gilead," "we," or "us") handles information from our callback request page at gileadmd.com, our Facebook and Instagram callback forms, and related follow-up. Where information is protected by HIPAA or other healthcare privacy laws, those requirements also apply. This online policy does not replace an applicable Notice of Privacy Practices. Contact our office for the notice relating to patient records and care.
Information we collect
Our Facebook and Instagram callback forms request your name, phone number and ZIP code, whether you have Maryland Medicaid coverage (Yes or No), and whether you currently receive therapy. You may also provide your email address, preferred callback time and communication preferences. Meta may prefill contact information from your account; check it before submitting.
Our website callback form also asks about Maryland Medicaid coverage and optionally whether you currently receive therapy.
We also keep information needed to handle your request, such as its submission time, source, follow-up status and contact attempts. The website and its service providers process technical information involved in displaying the page and submitting the form.
Beyond the Medicaid and therapy questions, please do not submit diagnoses, symptoms, medications, treatment records, insurance member numbers, Social Security numbers or other unrequested sensitive details through Meta forms or ordinary messages. These callback forms are intended for adults; call our office to ask about services for someone under 18.
How we use information
We use information to respond to your request, call you, consider your preferred callback time, identify the relevant service area and coordinate follow-up about our services. We use your Medicaid and therapy answers to guide follow-up about eligibility and services. We may use an email address you provide to respond about your request. We keep related records to manage inquiries and communication preferences.
Submitting a request does not establish a provider–patient relationship, enroll you in services or guarantee eligibility, coverage or an appointment. These forms are not intended for emergencies.
Who handles your information
We use HighLevel, also known as GoHighLevel or LeadConnector, for forms and contact management. Callback lead information, including contact details and submitted Medicaid and therapy answers, is sent by email to Gilead at [email protected] and recorded in a Google Sheets intake spreadsheet for Gilead’s follow-up. Digital Growth Marketing Partners helps operate our advertising and intake systems and may access information for that work.
Meta receives the information you submit through Facebook or Instagram, including your contact information and Medicaid and therapy answers, and makes it available to Gilead or its authorized account managers. Meta’s own handling of information is described in its privacy policy: https://www.facebook.com/privacy/policy/
Website tracking
Our callback webpage includes the Meta Pixel configured to report page-view events to Meta. This technology supports advertising measurement and may involve cookies and other online identifiers. Meta’s privacy policy explains its processing and available choices. You can also manage cookies through your browser settings; limiting cookies may affect website features. These controls do not withdraw information already submitted in a callback form.
Contact choices
Submitting a callback form requests a phone call from Gilead. Where an optional text-message checkbox appears, it records your communication preference. The current website workflow sends an automated confirmation text after submission. The Meta callback workflow does not automatically send text messages. Call our office to change your contact preferences or ask us to stop following up about your request.
Retention and security
We retain inquiry information for as long as reasonably necessary to respond to and manage requests and meet applicable legal obligations, then delete it or dispose of it appropriately. Retention can differ for healthcare records subject to separate requirements.
No online transmission or storage system can be guaranteed completely secure. Call our office for the appropriate way to provide medical or identity documents.
Your requests and questions
Contact us to ask about your information, request access, correction or deletion, or change contact preferences. We may need to verify your identity. Applicable law and healthcare record requirements can limit some requests. If you disagree with our response, contact us to request further review.
